Care Coordinator

Help support patients as they transition from hospital to home after surgery. This role focuses on real-time care coordination, patient support, and smooth discharge operations.

Care Companion

Flexible / part-timeBirmingham, AL or Chicago, IL

Support patients after surgery by meeting them at discharge, riding home with them, and helping ensure they get inside safely. This is a high-trust, patient-facing role.
