Help support patients as they transition from hospital to home after surgery. This role focuses on real-time care coordination, patient support, and smooth discharge operations.
What you'll do
- Respond to inbound requests from patients or families
- Collect and verify key discharge details
- Manage journeys in the ops dashboard
- Coordinate transportation manually
- Communicate with companions, patients, and families
- Escalate urgent issues when needed
Who we're looking for

- Highly organized and detail-oriented
- Clear communicator
- Comfortable using simple digital tools
- Healthcare, care coordination, CNA, EMT, or nursing student background is a plus
